Job Title: Bilingual Spanish Nurse Practitioner / Physician Assistant 

Location: Chula Vista, CA (In-Office, Minimal Travel for Occasional Home Visits) 

Employment Type: Contract, 11 Months (Part-Time) 

Start Date: December 2024 

About the Position 

One of our largest clients is seeking a nurse practitioner or physician assistant who is dedicated to improving the lives of older adults by helping them live independently with dignity. Through house calls and in-office wellness visits, they deliver comprehensive healthcare services to support homebound older adults with complex medical needs. We are currently looking for four compassionate, bilingual Spanish-speaking Nurse Practitioners or Physician Assistants to join our team for an 11-month contract, starting December 2024, with services going live on January 2025. 

Position Details: 

  • Part-time: 24 hours per week (3 days, 8-hour days) 

  • Flexible schedule options: 

  • Mon, Wed, Fri (24-29 hours) 

  • Tues, Thurs, Fri (24 hours) 

  • Flexible schedule within Mon-Fri based on business needs 

Responsibilities 

  • Conduct Medicare Wellness Visits at partner clinics and occasional home visits in the area. 

  • Assess patient medical history, review providers and prescriptions, and perform standard health measurements. 

  • Provide personalized health guidance and risk assessment. 

  • Communicate effectively with patients, simplifying complex medical information with care and compassion. 

  • Educate patients on health maintenance, illness prevention, and chronic health management. 

  • Offer referrals and resources as appropriate. 

  • Utilize the EMR system for timely and accurate documentation (EClinicals). 

  • Ensure work aligns with HCC coding standards. 

Qualifications 

  • Graduate of an accredited Nurse Practitioner or Physician Assistant program. 

  • Active, unrestricted California NP or PA license (DEA license required). 

  • Minimum of 2 years of experience in primary care, hospice, or urgent/outpatient care. 

  • Proficiency with EMR systems (EClinicals preferred). 

  • Knowledge of HCC coding. 

  • Required bilingual fluency in Spanish. 

  • Valid driver’s license, auto insurance, and reliable transportation.
